                This is a product testing session (the "Session") for the products of SachiSkin Pte Ltd (the "Company") which are still not fully developed or finished in its manufacture/ in the stage of laboratory testing/ still in the stage of research and development (the "Products"). Please sign below to indicate your acceptance of the terms and conditions in relation to your participation in this Session.

                1. Purpose: To test the reaction/feel/experience of the Participant's skin when the Products are applied onto the Participant's skin (face), and in particular, whether there is improvement in radiance, hydration and visible quality in the skin’s discolouration, pigmentation and skin smoothness for Well Ageing Formula 9131. 
                2. Procedure: The Participant agrees to apply the relevant Products which are under development onto the Participant's skin (face) for a flexible period of 20-40 days starting from the date you apply the Product (the "Trial Period"). To see great results during the Trial period, the Product needs to be applied both AM and PM after any serums. To be sure of product sample results we would advise no new product should be introduced to the skin during the trial time and routine should remain consistent until after the trial is complete. When introducing any new trial product to the skin a 48 hour patch test must be conducted to test for any adverse reactions, if any adverse reactions are observed please revert back to the coordinator of the trial to determine the next step and before continuing with the trial. 
                3. Potential Risks and Discomforts: The individual raw ingredients in the Products you will test are substances whose safety has been established. At the concentrations used in these products, they are not expected to cause irritation. However, it is possible that some subjects may experience irritation and/or dryness of the skin. Allergic reactions (for example, hives) are also possible. If you have a known allergy or sensitivity to any ingredients (product will contain cosmetic grade soothers and barrier repair ingredients), you should not participate in this Session. If you are pregnant you should not participate in this session or if you have any other ongoing health issues you should not participate in this session. By using the Products as instructed, your skin condition may improve, however, improvement is not guaranteed. Your skin condition may not change, or may even worsen, although this is unlikely.
                4. Confidentiality: The Participant hereby acknowledges and agrees that the information disclosed by the Company to the Participant in this Session is confidential information which is proprietary to the Company (the "Confidential Information"). The Confidential Information is being made available to the Participant solely for the purpose of enabling the Participant to participate in the Session. Accordingly, the Participant agrees not to use or disclose such Confidential Information to any other person. The Participant may not share the Product Name, or other information specified to any other person. 
                5. Consent: The Participant hereby agrees to participate in this Session subject to the terms and conditions set out herein. The Participant understands that the Participant's records and information received by the Company (the "Data") pursuant to the Participant's participation in 
                this Session will be kept confidential in accordance with applicable law. The Participant hereby agrees that any Data, photography taken by the participant for skin analysis may be used by the Company for the purposes of enabling the Company to carry out research and development and commercialisation activities, for the Company's product development purposes and for the purposes of statistical studies and marketing of the said product. The Company may also disclose the Data to related companies, its subsidiaries and affiliates and business partners for the purposes of carrying on the business of the Company and also to relevant government regulatory authorities upon their request in order to comply with statutory requirements.                 6. Waiver and Release: The Participant hereby acknowledges that his/her participation in the Session and the Participant's use of the Products exposes him/her to a possible risk of personal injury and that the Participant's participation in the Session and use of the Products are at the Participant's own risk. The Participant hereby releases and discharges the Company and its respective officers, directors, employees, partners, agents, licensees, subsidiaries, consultants, affiliates, members, managers, independent contractors and affiliates (collectively, the "related parties"), from any and all claims and liabilities for property damage, loss or injuries to the Participant or others, or any other claims, demands, disputes, causes of action of any kind arising from or in connection with the Participant's participation in the Session and the Participant's use of the Products, including claims that are known or unknown, foreseen or unforeseen, past, future or contingent or third party claims. The Participant agrees that the Participant shall not now or at any time in the future, directly or indirectly, commence or prosecute any action, suit or other proceeding against the Company and its related parties in connection with or arising from the Participant's participation in the Session and the Participant's use of the Products. The Participant hereby waives all actions, causes of action, claims and demands that may be brought against the Company and its related parties in connection with or arising from the Participant's participation in the Session and use of the Products. In the event that the Participant is injured through his/her participation in the Session or use of the Products, the Participant agrees to assume any and all financial obligation, either through the Participant’s personal health/ medical insurance, or through some other means for any medical costs which the Participant incurs due to any injury. 
                7. The Participant acknowledges and agrees that he/ she has read and fully understands this Form and that the terms and conditions set out herein shall be binding on the Participant, and his/her legal representatives, heirs and successors and assigns. 
                8. If any provision herein is held to be invalid or unenforceable, it shall not affect the validity or enforceability of any other provision. This Form constitutes the entire agreement between the parties herein and supersedes any and all prior oral or written agreements or understandings between the parties concerning the subject matter of this Form. 
                9. This Form shall be governed by and construed in accordance with Singapore laws. The Company and the Participant hereby submit to the exclusive jurisdiction of the Singapore courts.
